Eligibility for CCAC In-Home Services

 

All services provided by CCACs are fully funded by the Ministry of Health and Long Term Care. Services are provided using provincial guidelines. To be eligible for in-home CCAC services you must have

  • A valid Ontario Health Care

  • Needs that connot be met on an outpatient basis

  • A medical condition that can be adequately treated in the home

  • A need for at least one professional or personal support service

  • A willingness to participate in the home care plan

Each year CCACs are awarded very specific amounts of funding to provide services to

clients. They work to serve as many needs as they can with the resources provided. High service demand and not enough resources sometimes require CCACs to prioritize

client services into wait lists. Wait lists may also occur when there are shortages of trained personnel to fill positions such as nursing, speech/languages services and occupational therapy. A CCAC Case Manager assesses you for eligibility. Once eligibility has been established and the service is available, there is no cost to the client.
